Mission Possible Care Services

Community Access | NDIS Support

Getting out. Getting connected.

Community access through sport, adventure, and real-world experiences that build positive social connections. Not a group program. Not a day centre. 1:1 mentoring that gets participants out of their postcode and into the community.
Sessions are shaped around each participant’s interests, goals, and comfort level. The mentoring is the constant. The activities flex to the person.

Best fit

Who community access suits best.

This service works best for young men who are socially isolated and need a structured, consistent way back into community.

  • Socially isolated — haven’t engaged with community in months or years
  • Cycled through multiple support workers without connection
  • Interested in sport, fitness, or outdoor activities
  • Need a first step before group-based programs
  • Respond better to doing than sitting and talking
Common presentations

Psychosocial

Anxiety, depression, PTSD, ADHD — often co-occurring

Intellectual disability

Mild to moderate, often picked up late

Justice involvement

Youth Justice Orders, bail, recently released

Substance use

Often part of the picture, rarely the primary issue

The difference

What community access changes.

WITHOUT STRUCTURED SUPPORT

Disconnected

  • Haven’t left their immediate community in years
  • Cycled through multiple support workers
  • Limited or no positive social connections
  • Day-to-day routine that reinforces isolation
  • No structured activities or sense of belonging
WITH COMMUNITY ACCESS SUPPORT

Connected

  • A place to belong — a team, a gym, a regular activity
  • A consistent, trusting relationship with a mentor
  • Positive social networks in pro-social spaces
  • Regular outings that build confidence and routine
  • Skills and connections that last beyond sessions
Participant outcome

“He hadn’t left his postcode in over two years. After six months of weekly sessions, he’s playing futsal twice a week and just enrolled in a cert course.”
